I, as the parent or legal guardian of the participant, give permission for them to participate in pickleball activities organized by Aspen Pickleball. I understand that participation in sports involves inherent risks, including but not limited to injury, illness, or property damage. I certify that the participant is physically able to participate and has no known medical conditions that would prevent safe involvement.
I acknowledge and accept all risks associated with participation and agree, on behalf of myself and the participant, to release and hold harmless Aspen Pickleball, its owners, coaches, staff, independent contractors, and affiliated facilities from any and all liability, claims, or damages arising from participation. I also agree to abide by all rules, policies, and instructions provided by Aspen Pickleball and its staff.
